Three recent colored photographs with white background (size: 45mm X 35mm).

Note Verbale from Ministry of Foreign Affairs of Indonesia.

Vaccination Certificate.

Remarks: Concerned visa issuing authority will issue visa for the required duration with necessary number of entries
