Passport with minimum 6 (six) months validity.

Three recent colored photographs with white background (size: 45mm X 35mm).

Note Verbale from Ministry of Foreign Affairs of Indonesia.

Vaccination Certificate.

Remarks:

• Visa issuing authority will issue visa for the required duration with necessary number of entries on the basis of note verbale and scrutiny of the papers supporting purpose of visit.

In case of need of extension of the duration of visa the Passport and Immigration Directorate extend based on recommendation of the concerned Ministry/ host organization.
